We want to record the Adoption Lead and also record where (the physical location) the Adoption was completed and be able to report on that. We want to pull a report that says how many adoptions were completed at Petco, Petsmart, etc for grant reporting for those stores.

I think the Adoption Lead should be reported under the Adoptions Table, not the Animal table, since an Animal can be Adopted multiple times, and then that would not be accurate historically.
