Additional fields in "Adoptions" function

      1. Add "Adoption Location" with custom locations so we can differentiate location adopted from (WHICH Petsmart or Petco or other store) from the lead
      2. Add "Adoption Counselor" to adoption screen, with link to contact list
      3. Add "Follow Up" fields, with comments section (1st week follow up, 2nd and 3rd if possible)
